Service Coordinator
Responsible for coordinating services for all adult individuals receiving services through State General Fund (SGF), Developmental Disabilities Waiver (DDW), and other funded services on a caseload. Communicates with various departments within the agency as well as outside agencies to ensure individuals are receiving the highest level of quality services. Plans and attends meetings for each individual; assures paperwork is completed and submitted in a timely manner; manages conflict and issues that arise in a professional manner. Must demonstrate the following characteristics: good interpersonal skills to interact effectively and professionally with a wide range of people; excellent customer service with all internal and external customers; communicate effectively verbally and in writing to multiple levels of staff and stakeholders; understand the importance of urgency and accuracy; and be able to adapt to changing work conditions. MINIMUM REQUIREMENTS:
- Bachelor’s degree; or Associate’s degree with 2 years of experience
- Ability to pass background screening
- Ability to pass drug screening
- Possess a working telephone
- Must be able to type with speed and accuracy
- Possess a current and valid driver’s license
- Possess and maintain an excellent driving record
- Possess reliable transportation
